MOSES LAKE YOUTH BASEBALL 2013 PLAYER REGISTRATION FORM

Is your child playing Youth Baseball? Sign ups are tonight at the The Larson Rec. Center (batting cages) from 5:30pm- 7:00pm. Please print out and fill out the form below and have it ready tonight.

All participants please bring a copy of your birth certificate to registration, we need to update our

copies, and we are required to have them on file for new participants to be eligible for play.

This year's registration fees are as follows:

1st Child $50.00 2 Children $100.00 (additional children after the 2nd child will be an additional $30.00)

*There will be a $10.00 late fee per child that signs up after March 21

*All fee's and annual fundraiser monies are used to cover league cost, league insurance, charter fees, equipment & uniform fees, and umpires.
