REQUEST FOR QUALIFICATIONS

Grant County is soliciting developmental disabilities Employment and Day Program service providers to be added to the list of Grant County Developmental Disabilities Qualified Provider list. Current providers need not respond unless they wish to expand the scope of service.

Submittal requirements for the Request for Qualifications may be obtained at:

(1) The office of Grant Integrated Services located at 840 E. Plum Street, Moses Lake, WA 98837; or

(2) From the Grant County website at www.grantcountyweb.com.

The deadline for responses is 4:30 p.m. on May 15, 2013. An original proposal and two copies must be submitted as shown below in a sealed envelope clearly marked with the project title: “EMPLOYMENT AND DAY PROGRAM SUBMITTAL”. Late proposal and/or proposals submitted via e-mail or facsimile will not be accepted.
