SUMMONS AND NOTICE OF HEARING RE: TERMINATION OF PARENT-CHILD RELATIONSHIPS

_______________________________________________________________________________

TO:        RICHARD PEREZ

AND TO:    Any person claiming a parental interest in the above-named child

A Petition for Relinquishment and Termination of Parent-Child Relationships and for Adoption, which includes a request for termination of the parent-child relationship between the child and you and between the child and any other person claiming a parental interest in the child has been filed in the above-entitled court. The Petitioners are asking the court for an order permanently terminating the parent-child relationship between you and the child, permanently terminating all of your rights to the child, and for a Decree of Adoption declaring the Petitioners to be the legal parents of the child.

The child is was conceived approximately between June 1, 2010 and June 7, 2010, in Moses Lake, Washington. The child is expected to be born on or about February 23, 2011.     If the child is an Indian child as defined by the Indian Child Welfare Act of 1978, 25 U.S.C. 1901 et sec., and if you acknowledge paternity of the child or if your paternity is established prior to the termination of the parent-child relationship, your paternal rights may not be terminated unless you give valid consent to termination, or your parent-child relationship is terminated involuntarily pursuant to chapter 26.33 or 13.34 RCW.

Dated this __9th____ day of February, 2011 at Seattle, Washington.
