the necessary labor, materials, equipment, tools, and guarantees
thereof to construct Well No. 6 Reconstruction.

CITY OF WARDEN
WELL NO. 6 RECONSTRUCTION
Sealed Proposals will be received by the undersigned at the City of Warden, 201 South Ash Avenue, Warden, Washington 98857 up to 2:00 P.M.; local time on Thursday, December 9, 2010 for furnishing the necessary labor, materials, equipment, tools, and guarantees thereof to construct Well No. 6 Reconstruction.
This project provides for two (2) schedules of work. The Work shall include, but not be limited to, the following:
Schedule A – Well No. 3 Decommissioning
1. Furnishing all labor, material, and equipment necessary to remove and haul to waste deep well vertical turbine pump, lineshaft, column pipe, discharge pipe, valves, and appurtenances from Well No. 3, downhole video inspection of Well No. 3, and decommissioning of Well No. 3 in accordance with the Plans and Specifications, the Revised Code of Washington (RCW), and the Washington Administrative Code (WAC).
2. Demolition of Well No. 3 Well House.
3. Site cleanup and restoration.
Schedule B – Well No. 6 Reconstruction
1. Furnishing all labor, material, and equipment necessary to remove deep well vertical turbine pump, motor, lineshaft, column pipe, inflatable packer, discharge head and appurtenances from Well No. 6 and downhole video inspection and alignment test of Well No. 6.
2. Salvage of equipment and appurtenances from the Well No. 6 well house in accordance with the Plans and Specifications.
3. Reconstruction of Well No. 6 in accordance with the Plans and Specifications, the RCW and WAC, to include, but not be limited to the following:
• Furnish and install pea gravel, sand and temporary concrete cap from bottom of borehole to approximately 685 feet below ground surface (bgs).
• Perforation of existing 16-inch steel casing and furnish, install, and seal 12-inch diameter steel casing to approximately 685 feet bgs.
• Removal and disposal of pea gravel, sand and temporary concrete cap from bottom of borehole to approximately 685 feet bgs.
• Alignment test and downhole video inspection of the reconstructed Well No. 6.
• Material sampling, well development and disinfection.
• Providing of a well pump capable of up to 1,800 gpm for the step test and 1,500 gpm for the constant rate well performance test.
• Provide method(s) to dispose of all drilling fluids and test water.
• Site cleanup and restoration.
It is the intent of the Owner to award the project based on the lowest responsive, responsible bid received for both schedules combined. Bidders are required to submit a bid on both schedules. Failure to bid both schedules shall make the bid unresponsive.
The Work for Schedules A and B shall be substantially complete within 85 working days after the commencement date stated in the Notice to Proceed. All bidding and construction is to be performed in compliance with the Contract Provisions and Contract Plans for this project and any addenda issued thereto that are on file at the office of the City Clerk, City Hall, City of Warden, Washington.
The Proposals will be publicly opened and read aloud shortly after the time and date stated above. Proposals are to be submitted only on the form provided with the Contract Provisions. All Proposals must be accompanied by a certified check, cashier’s check, money order, or bid bond payable to the “City of Warden” and in an amount of not less than five percent (5%) of the total amount bid.
Contract Provisions and Contract Plans may be examined at the office of the City of Warden,
City Hall, local plan centers in the project area, or the office of the Project Engineer, Gray & Osborne, Inc. A copy of the Contract Provisions and Contract Plans may be obtained, free
of charge, in electronic format (PDF on compact disk (s)) along with registration as a planholder only at the Yakima office of the Project Engineer, Gray & Osborne, Inc., 107 South Third
Street, Yakima, WA 98901, (509)453-4833. Request for Contract Provisions and Plans may be faxed ((509)453-5953) or emailed (yakima@g-o.com). Request must include company
name, physical address, phone and fax numbers, and email address. Registration as a panholder
is required to obtain Contract Addenda. A list of panholders will be made available, by
request, through the Project Engineer’s office, via the U.S. mail.
Financing of the Project has been provided by the City of Warden, Drinking Water State Revolving Fund, and the Washington State Department of Ecology. The City of Warden expressly reserves the right to reject any or all Proposals and to waive minor irregularities or informalities and to Award the Project to the lowest responsive, responsible bidder as it best serves the interests of the City of Warden.
