CITY OF OTHELLO

REQUEST FOR STATEMENT OF QUALIFICATIONS

WELL NO. 6 REHABILITATION

AND

PLAN FOR MAINTAINING ADEQUATE CAPACITY

The City of Othello is requesting qualified engineers to submit statements of qualifications to perform two scopes of work, which includes providing construction administration services for the rehabilitation of Well No. 6 and for completing a Plan for Maintaining Adequate Capacity (PMAC) for the Wastewater Treatment Facility. The project budget for rehabilitating Well No. 6, including construction, is $312,000.00, with an anticipated construction completion date of February 2010. The project budget for the PMAC is $16,000.00, with an anticipated Plan completion date of April 2010. The criteria categories used in the selection process, weighted as indicated, will include: Qualification of key personnel (2); Relevant experience as demonstrated on previous projects (2); Previous performance and familiarity with the City (1); Expressed interest in the project (1); and Washington State Certified Minority Enterprise Participation (0.5).

Firms desiring consideration shall submit a complete qualification package and any other pertinent data to further assist the selection committee in evaluating the firm's qualifications to Mr. Jay Van Ness, Municipal Services Coordinator, 500 East Main Street, Othello, WA 99344. Four copies of the qualification package should be submitted to arrive no later than 5:00 PM (local time) on December 3, 2009. The City may choose to select a firm based on submittals received, or proceed with interviews.

The rehabilitation of Well No. 6 is funded through the Washington State Community Development Block Grant Program Recovery Grant with federal funds provided by the U.S. Department of Housing and Urban Development. The PMAC is funded through the Washington State Community Development Block Grant Program with federal funds provided by the U.S. Department of Housing and Urban Development. As a result, cost plus percentage of construction cost contracts will not be allowed and a number of state and federal equal opportunity and affirmative action requirements will apply to the selection process and conduct of both projects.

MITIGATED DETERMINATION OF NON-SIGNIFICANCE

PURSUANT to R.C.W 43.21C, NOTICE IS HEREBY GIVEN THAT on November 19, 2009 the Moses Lake Irrigation and Rehabilitation District (LEAD AGENCY) did issue a mitigated determination of non-significance relative to the environmental impact of the following described proposal:

The Moses Lake Irrigation and Rehabilitation District proposes to remove sediment in the Parker Horn area of Moses Lake to reduce impacts of the accumulated sediment. The primary project area of Parker Horn would be north of Neppel Crossing; however, minor dredging is proposed south of the crossing at the District's existing water pump intake and west of the bridge on the north shore of the lake.

The sediment would be dredged with a floating hydraulic dredge equipped with a 10-inch centrifugal pump and 20 ft boom known as a ladder. The sediment would be collected by the head on the end of the ladder and pumped through a completely contained temporary pipe that would deliver the sediment to upland disposal basins for dewatering and reclamation. Dewatering would occur on the waterward side of the railroad tracks on parcel 110475013. The temporary pipe would travel under Neppel Crossing during sediment removal west of the crossing.

The dewatering site would be prepared to receive the sediment by grading the property to make depressions or "cells" into which the water laden sediment would be pumped for dewatering. During dewatering, the lake water would return to the lake through subsurface flow. After the water percolates into the ground, the dry sediment would be piled up in berms around the perimeter of the cells to receive additional sediment pumped from the dredge. No surface water would leave the project site. Once each individual cell is full and the sediment has been dewatered, the dry material would be transported to a stockpile area. Dewatered sediment would be removed from the site as needed by dump trucks that would haul from the project site and connect with East Broadway Avenue.

The sediment stockpiles and haul road would be watered as necessary to control dust. Water for dust control would be pumped from Moses Lake and sprinkled on the appropriate areas.

The lead agency for this proposal has determined that it does not have a probable significant adverse impact on the environment subject to the stipulated mitigations under the State Environmental Policy Act Rules. The lead agency has determined that there are no probable significant adverse impacts that cannot be mitigated and has issued a Mitigated Determination on Non-significance. An environmental impact statement (EIS) is not required under RCW 43.21C.030(2)(c). This decision was made after review of a completed environmental checklist and other information on file with the lead agency. This information is available to the public on request and may be reviewed at the Moses Lake Irrigation and Rehabilitation District, 1055 W. Broadway Ave., Moses Lake, WA. 98837 during normal business hours.
