CITY OF OTHELLO

REQUEST FOR STATEMENT OF QUALIFICATIONS

WELL NO. 6 REHABILITATION

AND

PLAN FOR MAINTAINING ADEQUATE CAPACITY

The City of Othello is requesting qualified engineers to submit statements of qualifications to perform two scopes of work, which includes providing construction administration services for the rehabilitation of Well No. 6 and for completing a Plan for Maintaining Adequate Capacity (PMAC) for the Wastewater Treatment Facility. The project budget for rehabilitating Well No. 6, including construction, is $312,000.00, with an anticipated construction completion date of February 2010. The project budget for the PMAC is $16,000.00, with an anticipated Plan completion date of April 2010. The criteria categories used in the selection process, weighted as indicated, will include: Qualification of key personnel (2); Relevant experience as demonstrated on previous projects (2); Previous performance and familiarity with the City (1); Expressed interest in the project (1); and Washington State Certified Minority Enterprise Participation (0.5).

Firms desiring consideration shall submit a complete qualification package and any other pertinent data to further assist the selection committee in evaluating the firm's qualifications to Mr. Jay Van Ness, Municipal Services Coordinator, 500 East Main Street, Othello, WA 99344. Four copies of the qualification package should be submitted to arrive no later than 5:00 PM (local time) on December 3, 2009. The City may choose to select a firm based on submittals received, or proceed with interviews.

The rehabilitation of Well No. 6 is funded through the Washington State Community Development Block Grant Program Recovery Grant with federal funds provided by the U.S. Department of Housing and Urban Development. The PMAC is funded through the Washington State Community Development Block Grant Program with federal funds provided by the U.S. Department of Housing and Urban Development. As a result, cost plus percentage of construction cost contracts will not be allowed and a number of state and federal equal opportunity and affirmative action requirements will apply to the selection process and conduct of both projects.
