There has been filed with the Clerk of the above-named court a Petition for Relingquishment of the above-named child to the Petitioners for Adoption, and praying that the parent/child relationship between the alleged father and the above-named child be terminated.

You are hereby summoned to appear on the 7th day of April, 2006, at 9:00 a.m. at the King County Superior Court, Room W325, 516 Third Avenue, Seattle, WA 98104, and defend the above-entitled action, and answer the Petition for Relinquishment and Termination of Parental Rights, and serve a copy of your answer upon the undersigned attorneys for Petitioner at the office below stated.

If you fail to do so, judgment may be rendered against you according to the request of the Petition.

The child was conceived on or about June 2005 in the area of Moses Lake, Washington. The child's expected date of birth is March 19, 2006. The child's mother is Andrea Johnson.

YOUR FAILURE TO APPEAR AT THE HEARING MAY RESULT IN A DEFAULT ORDER PERMANENTLY TERMINATING ALL YOUR RIGHTS TO THE ABOVE-NAMED CHILD.

Any non-consenting parent has a right to be represented by an attorney, and an attorney will be appointed for an indigent parent who so requests.

You are further notified that your failure to file a claim of paternity within 30 days of the first publication is grounds to terminate your parent/child relationship with respect to the child, and such relief will be requested at the court hearing stated above.

One method is to file your response with the Clerk of the Court and serve a copy on the Petitioners' attorney by sending the response by Certified Mail with Return Receipt Requested.
