(Everett) - August 25, 2006 - Compass Health is issuing this advisory in connection with a security incident involving the potential disclosure of personal information on June 28, 2006. The incident involves a stolen laptop computer containing personal health information and social security numbers of a limited number of persons served by Compass Health, bridgeways, Catholic Community Services, Lake Whatcom Residential and Treatment Center, SeaMar, Snohomish County ITA, Volunteers of America or Whatcom Counseling and Psychiatric Clinic since October 1, 2005. Notification letters were previously sent via U.S. Mail to potentially affected persons for whom address information was available. This advisory has been sent, in accordance with RCW ch 19.255, to alert other potentially affected persons for whom complete contact information is unavailable. There is no indication that the data maintained on the laptop has been used for identity theft or other unauthorized purposes. Additional information can be found at www.compasshealth.org.
