You are hereby summoned to appear within twenty

five days after the date of first publication of this summons, and defend the above-entitled action in the above entitled court, and answer the Petition of the Petitioner, and serve a copy of your answer upon the undersigned Lawyer for Petitioner at the office below stated; if you fail to do so, judgment may be rendered against you according to the request of the Petition which has been filed with the Clerk of the court.

YOU ARE HEREBY NOTIFIED that there has been filed in this court a Petition praying that the parent-child relationship between parents of the above-named child and the above-named child be terminated. The object of the action is to seek an order relinquishing the child to the Petitioners for adoption and to terminate the parent-child relationship.

The child, Dominic Hernandez, is the son of Kimberly Hughes, and his father is Frank DeLeon. Dominic was born on Septmeber 8, 2003, at Deaconess Medical Center in Spokane Washington. The child is in the custody of Jody and Shelly Ramsey

The court hearing on this matter shall be on the 30th day of September, 2005, at 9:00 a.m. in Grant County Superior Court; address:

YOUR FAILURE TO APPEAR AT THE HEARING MAY RESULT IN A DEFAULT ORDER PERMANENTLY TERMINATING ALL OF YOUR RIGHTS TO THE ABOVE-NAMED CHILDREN

You are further notified that any non-consenting parent or alleged father has a right to be represented by an attorney, and an attorney will be appointed for an indigent parent who requests an attorney.

You are further notified that your failure to file a claim of paternity within twenty five days of the personal service of this notice or to respond to the petition within twenty five days of the first publication of this notice is grounds to terminate your parent-child relationship with respect to the child named above.

One method of filing your response and serving a copy on the petitioner is to send it by certified mail with return receipt requested.
